SurgHome is a post-operative recovery monitoring service operated by Heirloom Data, Inc. ("we," "us," or "our"). The service delivers automated SMS check-in messages to surgical patients during the 30 days following their procedure. The service screens for post-operative complications, tracks recovery milestones, and connects patients to their clinical care team when concerns are identified.
SurgHome is a screening and communication tool. It is not a substitute for professional medical care, diagnosis, or treatment. The service does not provide medical advice. If you are experiencing a medical emergency, call 911 immediately. Do not rely on this service for emergency medical needs. Always follow up with your surgeon and care team for medical decisions.
SurgHome is available to adults aged 18 and older who have undergone a surgical procedure at a participating facility and who have a mobile phone capable of sending and receiving SMS text messages.
By enrolling in SurgHome, whether through verbal consent at your care facility, by texting JOIN to our program number, or through another enrollment method, you consent to receive automated SMS messages related to your post-surgical recovery. Enrollment is voluntary. You may withdraw your consent and opt out at any time by replying STOP to any message.
You will receive approximately 6 scheduled check-ins over the 30-day post-operative period. Additional messages may be sent based on your responses or clinical need. Message and data rates from your mobile carrier may apply. We are not responsible for charges from your carrier. Message frequency varies. Reply STOP to cancel. Reply HELP for assistance.
You agree to provide accurate information during enrollment and when responding to check-in questions. You understand that your responses may be reviewed by clinical staff and may result in follow-up contact from your care team. You understand that failure to respond to check-in messages does not replace your obligation to seek medical care if you experience symptoms or complications.
Your use of SurgHome is subject to our Privacy Policy, available at surghome.com/privacy.html. We handle your information in compliance with HIPAA and applicable privacy laws.
SurgHome is provided "as is" as a supplementary monitoring tool. We make no warranties, express or implied, regarding the service's ability to detect all complications or replace clinical judgment. To the fullest extent permitted by law, Heirloom Data, Inc. shall not be liable for any indirect, incidental, consequential, or punitive damages arising from your use of the service. Nothing in these terms limits liability for gross negligence or willful misconduct.
We may update these Terms of Service from time to time. If we make material changes, we will notify active participants via SMS. Continued use of the service after changes constitutes acceptance of the revised terms.
You may terminate your participation at any time by replying STOP. We may discontinue the service or terminate your participation if we determine it is clinically appropriate or if you misuse the service.
These Terms of Service are governed by the laws of the State of Delaware, without regard to conflict of law principles.
